RESUMO

OBJECTIVE: To describe the experience of implementing social robotics as an adjuvant during the hospitalization process in pediatric oncology patients. METHODS: Before and after cohort study, applying an intervention with the Lego Mindstorms EV3 kit in patients between 8 and 17 years old that are hospitalized with a cancer diagnosis. We excluded patients from the intensive care unit or when their treating physician recommended so. The intervention consisted of a three-phase workshop: an open architecture story, building a car robot using the Lego Mindstorm EV3 kit, and cooperative playing activities such as races and passing obstacles. RESULTS: Thirteen patients received the intervention with robotic lego. The median age was 15 years (IQR = 3), and 84.6% of the population (n = 11) were male. We found significant improvement in the language (topic management p = .011 and communicative intention p = .034). Other characteristics improved, but not significantly (self-care activities index, catching). No adverse events occurred during the intervention. CONCLUSIONS: The results of this pilot study suggest that implementing social robotics during hospitalization in children with cancer is a therapeutic adjuvant and safe intervention that promotes better communication, self-care, and a physical activity improvement. For future studies, the impact of this intervention could be measured in hospitalized pediatric cancer patients.

El artículo presenta el caso clínico de un joven de 18 años de edad sin antecedente familiares de enfermedad crónica o hereditaria, nacido a término, sin complicaciones, quien desde los dos días de vida presentó una lesión tipo masa en la conjuntiva tarsal. Inicialmente, recibió tratamiento farmacológico tópico y seguimiento por oftalmología, por conjuntivitis bilateral persistente, masas tarsales recidivantes y cataratas en ambos ojos, por lo que requirió siete intervenciones con pobre respuesta al manejo farmacológico y quirúrgico. A los seis meses de vida se le diagnosticó hidrocefalia, que requirió manejo con derivación del ventrículo peritoneal. Dada la persistencia de la sintomatología y la refractariedad al tratamiento, se ampliaron los estudios y en una junta médica se sugirió el diagnóstico de conjuntivitis leñosa asociada a alteración del plasminógeno. Este diagnóstico fue confirmado por laboratorio clínico, que mostró sus bajas concentraciones de plasminógeno en muestras tomadas con intervalos de dos meses en tres ocasiones: 16,9%, 11,1%, 18,6% (valores de referencia: 70-150%). Se le indicó heparina de bajo peso molecular antes de procedimientos quirúrgicos mayores y triamcinolona tópica según síntomas oculares.


We present a case of an 18-year-old patient without a family history of ocular disease, born full term without complications, within his first 2 days a mass in the tarsal conjunctiva appeared. Initially he received topical treatment and follow-up by the ophthalmology department with a diagnosis of persistent bilateral conjunctivitis, relapsing tarsal masses and cataracts in both eyes requiring a total of 7 surgical interventions with a poor response. At the age of 6 months he was diagnosed with hydrocephalus and required a ventricular-peritoneal shunt. Giren the persistence of the symptoms, further studies were made and a medical board made the diagnosis of ligneous conjunctivitis associated to low levels of plasminogen. The diagnosis was confirmed by decreased levels of plasminogen in serum measured three times with 2 months intervals: 16.9%, 11.1%, 18.6% (reference valúes 70'150%). Low molecular weight heparin was ordered before surgical procedures, and topical triamcinolone applied according to ocular symptoms.

Introducción: El cáncer infantil ocasiona importante mortalidad. El objetivo es caracterizar y determinar la sobrevida de un centro especializado. Métodos: Análisis de supervivencia en menores de 18 años de edad, diagnosticados con cáncer del 01/10/2010 al 31/03/2016. Se analizaron variables sociodemográficas (edad, sexo, seguridad social y zona de residencia) y clínicas (tipo de cáncer y categorización del riesgo). Se utilizó el método de Kaplan-Meier y Log-Rank Test. Resultados: 135 pacientes, con una edad promedio 7,4 años (DE 4,84), la mayoría de sexo masculino y pertenecientes al régimen contributivo. Las neoplasias más frecuentes fueron leucemias y tumores de sistema nervioso central, predominancia de alto riesgo. La supervivencia global fue del 75 % y la supervivencia libre de evento fue del 60 %. Los tumores sólidos y de alto riesgo presentaron mayor mortalidad. Conclusiones: La supervivencia global a 5 años es similar a los datos internacionales. La mayoría de los tumores sólidos eran de alto riesgo, probablemente asociados a mayor tiempo de evolución.


Introduction: Childhood cancer is a major cause of mortality. The aim is to characterize the population and determine the survival of child population in a reference center. Methods: Survival analysis patients younger than 18 years diagnosed with cancer 01/10/2010 to 31/03/2016. Socio demographic variables (age, sex, social security and area of ​​residence) and clinical (type of cancer and risk categorization) were analyzed. The Kaplan-Meier and Log Rank Test was used. Results:135 patients were evaluated, the average age at diagnosis was 7,4 years (SD 4,84), mostly male, contributory scheme and urban residence. The most frequent neoplasms were leukemias and central nervous system tumors and most were high-risk. Overall survival was 75% and event-free survival of 60%. Solid and high-risk tumors had a higher mortality. Conclusions: The overall 5-year survival is similar to international data. Most solid tumors were high risk probably associated with a longer history.
